The first step is to answer some simple questions via our HIPAA secured online form. Once you complete some basic identifying information and tell us a bit about your medical history, we collect you’re payment information. You can schedule an appointment or request to enter our virtual online waiting room to be seen by a medical marijuana practitioner ASAP during our normal business hours. As of 03/20/2023, separate "medical cards" are no longer needed or being issued by NYS. The official medical certification obtained Dr. Dave will contain a registry ID barcode and is all you need to purchase medical cannabis.

In New York the maximum certification length is for 12 months. Patients are generally approved for the maximum time period (12 months) unless there is a medical reason for a shorter certification duration or closer follow up.

All you need is a government-issued ID such as a driver’s license or US passport. Records of your medical condition are also helpful but you can still be evaluated if they are not available.
