How can I get medical marijuana? Follow these 3 easy steps:

  1. Talk with your Health Care Provider (That’s us!)

    If your health care provider decides that medical marijuana is right for your qualifying condition, you will get a signed certification form. You need this form to apply for a registry ID card.   2. Apply for Registry ID Card Online

    For specific registration instructions, go to health.ny.gov/mmp and click on How to Register.

    To register, you must have this information ready: •

    • The certification form your health care provider gave you.

    • Proof of identity and New York State residency.

    • Parents or legal guardians of patients under the age of 18 must submit a copy of a government-issued ID and a copy of the patient’s birth certificate.

    You can:

    • Print a temporary ID card to use while you wait for your permanent registry ID.

    • Select up to two caregivers who can buy medical marijuana for you. They must register after your registration is approved.

    • Register by phone, but your registration may take longer.

  3. Visit a Medical Marijuana Dispensing Facility

    Once you have your temporary or permanent registry ID card, you can visit a New York State dispensing facility to buy medical marijuana. Bring your certification form and registry ID card with you.
