If your health care provider decides that medical marijuana is right for your qualifying condition, you will get a signed certification form. You need this form to apply for a registry ID card. To register, you must have this information ready: •
The certification form your health care provider gave you.
Proof of identity and New York State residency.
Parents or legal guardians of patients under the age of 18 must submit a copy of a government-issued ID and a copy of the patient’s birth certificate.
You can:
Print a temporary ID card to use while you wait for your permanent registry ID.
Select up to two caregivers who can buy medical marijuana for you. They must register after your registration is approved.
Register by phone, but your registration may take longer.
Visit a Medical Marijuana Dispensing Facility
Once you have your temporary or permanent registry ID card, you can visit a New York State dispensing facility to buy medical marijuana. Bring your certification form and registry ID card with you.
